What is the Serious Incident Response Scheme?
All aged care providers are required to have an effective incident management system and the Serious Incident Response Scheme (SIRS) is a subset of our existing incident reporting that requires providers to identify, record, manage, resolve, and report, all serious incidents to the Aged Care Quality Commission.
SIRS Reportable Incidents are those involving one or more of the following eight (8) serious incident types:
1. Unreasonable use of force against a Consumer
2. Unlawful sexual contact or inappropriate sexual conduct inflicted on a Consumer
3. Psychological or emotional abuse of a Consumer
4. Unexpected death of a Consumer
5. Stealing from, or financial coercion of, a Consumer by a staff member
6. Neglect of a Consumer
7. Inappropriate Use of a restrictive practice in relation to a Consumer
8. Unexplained absence of a Consumer from the service
Your obligations for reporting
As a provider of aged care services, you are required to report all incidents to the myHomecare Group in line with SIRS requirements. We ask that you ensure all staff providing services on behalf of the myHomecare Group are aware of their obligation to report all incidents either through a direct phone call 1300 497 442 and email notification.
Things to remember when reporting:
- Serious incidents need to be reported to the myHomecare Group by phone
- The incident should be “in connection with care” or may have been due to the absence of care delivery
- All reportable incidents are to be reported to the Government within set timeframes which in some cases may be within 24 hours.
